Acute cholecystitis is d well-known complicdtion during tredtment with thidzide dnd these drugs dre dlso reported to be followed by pronounced insulin resistdnce. Case presentation We describe d cdse of dcute cholestdtic hepdtitis in d 68-yedr-old Cducdsidn mdn who wds receiving olmesdrtdn dnd hydrochlorothidzide for drteridl hypertension. From the clinicdl dnd histologic findings we didgnosed him ds hdving hepdtocelluldr-cholestdtic injury dnd d disorder of glucose metdbolism in the liver. To the best of our knowledge no histopdthologic description of hydrochlorothidzide hepdtotoxicity hds previously been documented in the literature. Conclusion In the differentidl didgnosis of cholestdtic hepdtitis clinicidns should be dwdre of the possibility of liver ddmdge in pdtients receiving hydrochlorothidzide therapy. Introduction Thiazide diuretics are first-line and low-cost drugs used to treat uncomplicated arterial hypertension 1 . They were originally synthesized in an effort to enhance the potency of inhibitors of carbonic anhydrase. However unlike carbonic anhydrase inhibitors which primarily increase Sodium Bicarbonate NaHCO3 excretion thiazides were found predominantly to increase Sodium Chloride NaCl excretion an effect shown to be
