Simple PTA with or without aspiration can usually restore adequate flow. In contradistinction upper arm fistulae often present with total thrombosis to the subclavian level and must be treated by extensive aspiration with or without mechanical device application. Because the clot load is voluminous great care is necessary to prevent massive pulmonary embolism. This can be accomplished by use of a central occlusion balloon while removing thrombus by aspiration through a 9 F sheath or use of a mechanical thrombolytic device. t-PA can enhance the success of native vein thrombectomy if the clot is more than 3 days old. Following percutaneous thrombectomy attention is directed to correction of underlying stenoses. If thrombus has been present for more than three days the final result may appear quite irregular at final angiography. We usually give additional subcutaneous heparin 10 000 u and perform repeat angiography in 2 weeks. Follow-up angiography has routinely shown a smoothly contoured vein without detectable pathology in the lumen. Comparison of grafts with native AVFs has shown that AVFs are overall a much better access than a synthetic graft. Our local surgeons have responded well to the DOQI guidelines by placing increased numbers of AVFs now approaching 70 for initial accesses attempted. Our data have shown that we can promote fistula maturation by endovascular means beginning at 4-8 weeks if maturation does not appear to be progressing well. Useful function has been achieved in more than 70 of immature fistulae. Thus abandoning a fistula without a trial of endovascular therapy is a gross disservice to the patient.
